Event Description:
Shifts in the regulatory and payment environment for Medicare Advantage between 2023 and 2024 have put increasing financial pressure on organizations providing value based primary care. While many such organizations have scaled back their growth, closed primary care centers, or exited the VBC space altogether, Oak Street Health, a leading value based primary care organization for adults on Medicare, has continued to expand. Oak Street's continued growth is driven by a care model that reduces total cost of care and improves patient outcomes with "high doses" of primary care, enhanced management of chronic conditions, and wrap-around support for high-risk patients. Oak Street's care model also provides a path through which the organization can successfully navigate shifting regulatory and payment environments to continue finding the value in value-based care.
Dr. Rebecca Anastos-Wallen is a Primary Care Physician and National Medical Director of Population Health at Oak Street Health, a leading value-based primary care organization for adults on Medicare that operates over 200 primary care clinics in 28 states. In her role as National Medical Director of Population Health, Dr. Anastos-Wallen is the clinical lead overseeing the design and implementation of Oak Street Health's value-based care model. Her scope includes Oak Street Health's national work related to screening and diagnosis, chronic condition management, high risk patient management, specialty care and partnerships, and prevention of avoidable hospital admissions and readmissions. Dr. Anastos-Wallen also leads Oak Street Health's Central Telehealth and Podiatry clinicians and serves as a PCP for a panel of Oak Street patients in West Philadelphia. Dr. Anastos-Wallen completed her residency in Primary Care Internal Medicine at the University of Pennsylvania Hospital. She holds an MD from the University of Pennsylvania and a bachelor's degree in History from Yale University. Prior to medical school, Dr. Anastos-Wallen worked as a management consultant at the Boston Consulting Group for 5 years with a focus on global health.
